Welcome to Astra Veritas Ink. We are pleased that you have selected our studio for your tattoo experience. Please complete this form. Ensure that the photo of your ID is clear and legible, otherwise your consent form will not be accepted. All information is kept secure and confidential.

INFORMED CONSENT
By filling out the form below, I acknowledge and agree to the following:
Voluntary Procedure: I am voluntarily requesting a tattoo to be applied by a licensed tattoo artist at Astra Veritas Ink.
Legal Age: I affirm that I am 18 years of age or older, or I have parental/legal guardian consent if under 18, as allowed by Virginia Code §18.2-371.3.
Medical Risks: I understand the procedure involves risks such as infection, allergic reactions, scarring, and complications due to existing medical conditions.
Disclosure: I have truthfully disclosed all medical conditions and allergies that may affect this procedure to my artist.
Aftercare: I have received, read, and understand the aftercare instructions and will follow them to minimize the risk of complications.
No Guarantee: I acknowledge that no guarantees have been made regarding the results, appearance, or longevity of the tattoo.
Photography Consent: I DO consent to my tattoo being photographed and/or filmed for portfolio and/or social media purposes.
Release of Liability: I release the tattoo artist and Astra Veritas Ink from all liability for any injury, loss, or damage caused by the tattoo procedure, unless caused by gross negligence.
